This is the Special Request Form that is to be used by players if they require or have special requests such as a particular team, coach or player pairing that they feel is needed for the upcoming season. The requests will go to the relevant Youth Director, Junior Youth Director or the Mini Director depending on which division the request is being made for. The form is to be only used if needed. We will make every effort to honor the requests and make them happen, but in some circumstances it is just not possible to due to team make up and numbers and also balancing the teams in a particular division.
The form needs to be received prior to any separating or evaluating of players. Requests received after the team rosters have been divided will not be considered under any circumstances. We appreciate you all providing as much information as possible to aid in the decision to respond to the special request.
